Below is my module for scheduling, intake and health history forms. A card will be kept on file, though will not be charged if you choose to reserve now & pay later, when booking.
For clients seen and/or scheduled before June 24th, 2025: you will be seen at your previous rate, and not the rate published below. Please bring payment in-person instead of paying online.
